SUPPORTS COORDINATION

 

JOB DESCRIPTION

  1. NATURE OF WORK

 

A Supports Coordinator provides family-focused services to children with developmental disabilities for the purpose of assisting the child in building on his/her strengths and helping the family to remain safely together in the family home.  The Supports Coordinator will work in an environment convenient to the child and family to assist with linking, monitoring, and coordinating services. 

 

 

  1. DUTIES & RESPONSIBILITIES

 

  1. SPECIFIC:
  1. To provide support and resources to families and children diagnosed with developmental disabilities
  2. To effectively coordinate, educate and train family members and support systems on individual needs of each child and therapy systems.
  3. To monitor individuals a minimum of one time per month and for at least one hour for their progress on goals as well as to provide additional support and/or resources.          
  4. Work skillfully and tactfully with other people significant to the client including families, schools, doctors, and other systems within the client’s life.
  5. Timely completions of annual social assessments and person-centered plans, quarterly progress reports, and monthly progress notes.
  6. Completion of annual supports coordination profiles, respite profiles, and bi-annual CLS profiles.
  7. CLS aides are responsible for turning in CLS logs in person to the agency.  Faxed or emailed logs will not be accepted.
  8. Provide crisis intervention and on-call response services as assigned by supervisor.
  9. To maintain strict confidentiality regarding his/her work with clients of the agency.

 

  1. AUTO SAFTY EXPECTATIONS
  1. In transporting children, the Supports Coordinator will always follow the Michigan Transportation Law regarding children, which states:  "Children under one year of age must be secured in a safe child care seat." "Children between one and four years of age must ride in a safe car seat when they are front seat passengers and must use either the vehicle seat belt or a safe child car seat when they are back seat passengers."
  2. The Supports Coordinator will assure that all occupants of the car wear seat belts at all times.
  3. The Supports Coordinator will follow all traffic laws and use all necessary precautions to assure the safety of his/her passengers and themselves.

 

 

  1. ESSENTIAL JOB FUNCTIONS

 

  1. Ability to communicate verbally and effectively to exchange information with coworkers, peers, the community, and others involved in your position.
  2. Ability to maintain a stationary position for extended periods of time.
  3. Ability to be physically mobile when working with students.
  4. Ability to determine and assess job related documents and visually utilize Agency equipment.
  5. Ability to physically handle and perform job related functions, including the operation of Agency equipment.
  6. Ability to physically position oneself to effectively interact with students.
  7. Ability to ascend/descend stairs safely and successfully.
  8. Ability to lift and transport 40lbs.

 

 

  1. MINIMUM PERSONAL & PROFESSIONAL QUALIFICATIONS

 

  1. Bachelor’s degree in Social Work or a health-related field.
  2. Possess an understanding and sensitivity to the client’s cultural and socioeconomic characteristics.
  3. Knowledge of developmental disabilities.
  4. Knowledge of family relationships and child development.
  5. Good oral and written communication skills.
  6. Good organizational skills.
  7. Good computer skills.
  8. Valid driver’s license, adequate insurance with a car available at all times. Must have good driving record. The CLS aide’s personal vehicle is the expected primary mode of transportation in the course of performing job duties and responsibilities.  The vehicle must be kept in a safe driving condition. 
  9. Ability to work effectively with a variety of people both within and outside the agency.  Must promote a positive image for the agency in the community and in relationships with clients, involved professionals and others significant to services.
  10. Personal qualities of warmth, sensitivity and the ability to establish effective relationships and a capacity to make sound decisions.
  11. Knowledge of related community resources.
  12. Appropriate assertiveness, flexibility, judgment, self-organization and self-direction.
  13. High frustration/stress level.
  14. Positive, accepting attitude.
  15. Completion of a minimum of 24 hours of continuing education and trained related specifically to children and family issues each calendar year.
  16. Become familiar with and follow the code of ethics for your profession and agree to abide by the related Agency standards.
  17. Positive acceptance of supervision in order to learn and be trained in job-related functions.
  18. Personal qualities of organizational skills, assertiveness, good judgment, conscientiousness, attention to detail, and timeliness.
  19. Good moral character.
  20. A personal commitment to the Agency’s mission, philosophy, and values.
