Assess youth and families in order to develop, coordinate and implement individualized treatment plans with youth and their caregivers to provide stability in their placements and assure that their needs are being met; Coordinate youth’s placement within foster care, school and community, DHS and family court.  Manage a caseload of approximately eight children; Testify in court; Provide support and training to Treatment/Enhanced Foster Care Parents.  


  • Develop, coordinate and implement concise, accurate treatment plans with youth, and treatment/enhanced foster families that provide support and stability of placement for the youth
  • Provide and/or coordinate services to meet needs of youth in order to stabilize youth’s placement, school placement and reinforce school success. (e.g. parenting time, placement visits, visits birth parents, behavior specialist services, wraparound meetings and other case conferences.) 
  • Coordinate required medical and dental services for all foster children and monitor the implementation of these services by the caregiver
  • Coordinate all community and resource referrals and services to the youth and treatment foster care parents 
  • Prepare court reports and petitions and provide testimony to support permanency, safety, and well-being of the child and family, as required by DHS contract, DHS and agency policy, and court mandates
  • Provide emergency, on-call interventions on after hours, and on holidays, as required
  • Integrate learning from required DHS training into day to day responsibilities
  • Ensure transportation is provided or arranged for parents and foster children in order to meet all goals and objectives on the treatment plan
  • Plan and participate in special events for clients and caregivers
  • Complete data entry into all data management systems as required, as well as keep paper files accurate, complete, and in compliance with policy, rules, and state law
  • Coordinate with the Behavioral Aide on a weekly basis to implement and review all behavior plans and logs
  • Provide consistent support and training to treatment foster care parents
  • Communicate at least monthly with the child’s planning family and/or birth family in order to facilitate communication regarding the child’s needs
  • Lead treatment team meetings at minimum, once per month


  • Master’s degree in a Human Services field –OR–
  • Bachelor’s degree in a Human Service field with at least two years of experience working with children and families in a foster placement setting along with experience in assessment and treatment planning
