Responsible for developing and implementing therapeutic treatment plans with families that will assist them in resolving the issues that led to a referral to the child welfare system.  Use counseling techniques, community resources, concrete services, and family empowerment to assist the family in goal achievement.  Communicate effectively with the local DHS, courts, medical and education systems to advocate for the family.  Responsible for regular visits with the family in their home, administer required assessments and survey instruments; writing and submitting treatment plans, service plans, safety plans and court reports; assist in the arrangement of aftercare services based on families’ needs for a caseload of up to 12 families. Works a flexible schedule (including non-traditional hours) based on families’ needs.




  • Provide outreach-counseling services to high-risk families, assisting them to actively work to reduce risk factors and bring stability to the family
  • Assist families in obtaining concrete services such as housing, food, clothing, medical care, utilities, home maintenance and repair, etc.
  • Provide skills training including but not limited to; parenting, parental resilience, social connections and concrete support, child development, social and emotional competence of children, nurturing and attachment and infant safe sleep
  • Establish community contacts to refer families/make collateral contacts to services including but not limited to: Early on, domestic violence, mental health, substance abuse counseling, longer-term counseling; financial management; concrete services.  Teach families how to advocate for themselves with these resources
  • Maintain face-to-face contact with families according to the contract defined frequency/duration and the individual needs of the family
  • Prepare reports (treatment plans, services plans, court reports, closure summaries) within the appropriate time frames; maintain safety planning and documentation thereof throughout services according to the needs of the family
  • Observe and respond to warning signs of abuse and neglect
  • Maintain case files and documentation of client contact/activities to support accurate and timely billing procedures
  • Provide twenty-four (24) hour availability to clients and a flexible work schedule
  • Transport clients in your own vehicle



  • Masters degree in social work or related human service field
  • Experience in working with multi-problem families
  • Maintain or seek state license, certificate or registration commensurate with the function of the position and the individual's academic training and experience
